PURPOSETo plan, organize, develop, and direct the overall operation
of the Social Services Department in accordance withcurrent
federal, state, and local standards, guidelines, and regulations
along with established policies and proceduresand as may be
directed by the Administrator, to meet medically-related emotional
and social needs of the resident.Disclaimer: The following

management.ESSENTIAL FUNCTIONS1. Know, understand, incorporate, and
demonstrate the Mission, Vision, Values and Guiding Behaviors of
TrinityHealth and Trinity Senior Living Communities in behaviors,
practices, and decisions.2. Consistently demonstrate (leads by
example) Sanctuary Values to all internal and external customers
(residents,visitors, volunteers, and associates.) Create and
maintain an atmosphere of warmth, propagating a calmenvironment
throughout the community. Actively participates / intervenes with
direct care as needed and as a rolemodel in ongoing Sanctuary
Training of personnel.3. Execute assignments in a culture that is
shared and collaborative across all divisions within TSLC.4.
Establish and/or maintain departmental procedures that support the
necessary operational functions for the SocialServices
Department.5. Maintain department budget, place orders for
equipment and supplies as necessary.6. Determine departmental
staffing requirements and assist in the recruitment, interviewing,
and selection ofpersonnel for the Community or Department.7.
Monitor department work hours (including vacation and holiday
schedules), personnel (if applicable), workassignments, etc., to
ensure proper workflow and adequate staffing levels in compliance
with state minimumregulations and sanctuary budgeted PPD hours.8.
Evaluate, develop, mentor, coach, counsel and discipline department
staff. Supports community personnel fromother departments through
coaching and mentoring to help achieve optimum standard of
excellence. Addressesissues of concern through courageous
conversation and notifies department manager of any interactions
requiringattention.9. Participate in community surveys and when
necessary develop a plan of correction for department
deficiencies.10. Assume the authority, responsibility and
accountability of directing the social service department and its
programs.11. Participate in planning related to resident, family,
and community interests. Participate in discharge
planning,development and implementation of social care plans and
resident assessments. Perform administrative duties suchas
completing medical forms, reports, evaluations, studies, charting,
arranging transportation, etc., as necessary,coordinating
activities with other departments as necessary.12. Promotes optimum
transition of resident for appropriate and safe placement for
discharge. Provides ongoingcommunication with resident and
responsible persons of progress/status during stay.13. Provide
information to the resident/family on Medicare/Medicaid as well as
other financial assistance programsavailable. Provide consultation
to staff, community agencies, etc., in efforts to solve resident
concerns through thedevelopment of social service programs.14. Keep
abreast of economic conditions/situations, recommending adjustments
that assure continued provision ofdaily social services.Director

notes are informative and descriptive of the services provided as
well as theresident's response(s) to the service. Review and
develop a plan of correction for social service deficiencies.16.
Provides information and education for residents and responsible
person on Advanced Directives to supportinformational needs.
Collaborate with Interdisciplinary team related to advance
directives and assures medicalrecords are compliant with the
completion.17. Serve on, participate in, and attend various
committees of the community (i.e., Infection Control, Policy
Advisory,Pharmaceutical, MDQI, etc.) as required or appointed by
the Administrator/Executive Director.18. Provide written and/or
oral reports regarding social service programs and activities as
directed by suchcommittee(s). Conduct grievance tracking and report
out in the appropriate meeting. Evaluate and
implementrecommendations as they may pertain to social services.19.
Participate in resident/group council meetings as requested and
provide support services to such council.20. Forecast needs of the
department. Assist in preparing and planning the social services
department budget, makingadjustments as necessary. Maintain current
written records of department expenditures and cost reports.21.
Respect residents' rights, including maintaining the
confidentiality of all resident care information, knocking
beforeentering a resident's room, and ensuring all social services
personnel are knowledgeable of the residents'responsibilities and
rights including the right to refuse treatment. Review resident
complaints and grievances,making written/oral reports indicating
what action(s) were taken to resolve them.22. Investigate and
submit accident/incident reports to Human Resource within 24 hours
after occurrence.23. Must function independently, make independent
decisions, demonstrate flexibility, personal integrity and ability
towork effectively with residents and personnel.24. Report
occupational exposures to blood, body fluids, infectious materials,
and hazardous chemicals in accordancewith the community policies
and procedures.25. Maintains a working knowledge of applicable
Federal, State, and local laws and regulations, TSLC
CorporateIntegrity Program, Code of Ethics, as well as other
policies and procedures in order to ensure adherence in amanner
that reflects honest, ethical and professional behavior.26. Provide
residents with information concerning resident rights, living
wills, etc., explaining as necessary. Serve asgrief counselor to
families in need.MINIMUM QUALIFICATIONS1. Must possess a bachelor's
degree in social work or a human service field including but not
limited to sociology,special education, rehabilitation counseling,
or psychology along with one year of supervised social
workexperience in a health care setting working directly with
individuals.2. Must be licensed as a Social Worker in this State.3.
Must be knowledgeable of state regulations and guidelines related
to long-term care and specifically regardingsocial worker
responsibilities. (i.e. MDS, Discharge Planning, Advance
Directives, etc.)4. Previous experience working in long-term care
or with the geriatric population preferred5. Must have one to two
years' experience in a supervisory capacity in a hospital, skilled
nursing care community, orother related medical community6. Must
lead by example and possess superior customer service skills and
professionalism7. Flexibility to adapt to ongoing change and work
in a fast-paced, customer driven environment.8. Interpersonal
skills to drive collaboration, commitment and productivity when
working with cross-functionalteams, customers and end users.9.
Self-starter, highly motivated with high energy level.10.
Demonstrates superior written and verbal communication and
presentation skills.11. Basic to intermediate proficiency with
Microsoft product suite (MS Word, Excel, Power Point, etc.);
basicknowledge and experience with electronic mail and calendaring
system in PC LAN environment. Ability to typewith speed and

high degree of personal accountability, responsibility and
independent decision making abilities withthe skills to plan,
organize, develop, implement and interpret programs, goals,
objectives, policies and proceduresof the organization.13. Position
requires minimal travel (less than 10%) within the Community's
geographic region and to home office inLivonia, MI.PHYSICAL AND
MENTAL REQUIRMENTS AND WORKING CONDITIONS1. Must be able to adapt
to frequently changing work parameters.2. Must be able to see, hear
and smell or use prosthetics that will enable these senses to
function adequately so thatthe requirements of this position can be
fully met.3. Primarily works inside with frequent kneeling,
crouching, reaching, lifting, sitting, walking, bending and
lifting.Must be able to push, pull, move and or lift a minimum of
25 pounds to a minimum height of 3 - feet and beable to push, pull,
move and or carry such weight a minimum distance of 10 feet. May be
necessary to assist in theevacuation of residents in emergency
situations.Physical mobility that includes movement from place to
place.Physical agility, that includes ability to maneuver body
while in place.Dexterity of hands and fingers.Coordination,
including eye-hand, hand-foot.4. Must possess the above ability
with or without the use of prosthetics that will enable adequate
functionality so thatthe requirements of this position can be fully
met.5. Subject to exposure to noise, infectious waste, diseases,
conditions, etc. including TB, HIV, HEP B viruses. Maybe subject to
the handling of and exposure to hazardous chemicals.6. Must be able
to speak, read and write the English language in an understandable
manner.7. Must be able to relate to and work with ill, disabled,
elderly, emotionally upset and at times hostile individualswithin
the community. Must possess the ability to deal tactfully with
personnel, residents, family members, visitors,government
agencies/personnel and the general public.8. Must not pose a direct
threat to the health or safety of other individuals in the work
place.9. Must be able to cope with the mental and emotional stress
of the position.10. Must meet the general health requirements set
forth by the policies of this community, which include a medicaland
physical examination.11. May be required to work beyond normal
working hours, on weekends and other positions temporarily
whennecessary and may be subject to call back during emergency
conditions. May be required to work on shifts otherthen those which
