addition to our Care Management Team! In partnership with the
Senior Manager of Patient Programs, the Senior Care Management
Program Coordinator is responsible for the Care Management program
at Erie and other population health management initiatives,
including but not limited to agency-wide transitions of care and ED
navigation efforts. The Senior Program Coordinator utilizes their
clinical knowledge and program coordination skills to ensure that
these programs and activities are implemented and optimized in line
with both contractual requirements and Erie's standards for
high-quality, patient-centered care. The Senior Program Coordinator
leads the integration of these activities at the site levels with
the care teams and across the Patient Programs department and other
centralized teams at Erie.At Erie, we are proud to provide
