- JOB SUMMARY - Reports to the Director of Patient Care Services. The Stroke Program Coordinator will oversee the Stroke Patient Navigators and collaborate with the Stroke Program Medical Director for the system, implementation, coordination, and evaluation of clinical care provided to the stroke patient population as set by the guidelines formulated from the American Heart Association/American Stroke Association (AHA/ASA) and for achieving/maintaining stroke certification(s). Will provide in-depth knowledge of stroke care and procedures in order to drive systematic evaluation of stroke clinical care with an emphasis on patient safety and improving stroke care. Will review and analyze issues, identify opportunities for improvement, recommend corrective actions, and provide ongoing monitoring. Will utilize a standardized approach in evaluating the care delivery processes with an emphasis on decreasing variation and improving outcomes. - MISSION & VISION - Mission: To enhance the physical, mental and emotional well-being of the communities we serve as the community's provider of outstanding quality, superior value and comprehensive health care services. Vision: Our vision is to achieve: - Innovative health care and well-being services of the highest quality at the greatest value - Easy access and convenience - Outstanding patient experiences - Ongoing education involving physicians, patients and the community - JOB SPECIFICATIONS - Education and Experience - The knowledge, skills, and abilities as indicated below are normally acquired through the successful completion of a Bachelor's Degree. RN license in the State of Indiana is required. 3-5 years of nursing with practical experience in stroke, trauma and/or critical care nursing required. Additionally, one- two years in data management, outcome management and measurements experience preferred. Knowledge of AHA/ASA, Joint Commission core measures and coding required. Technical skills including, Microsoft office (Word, Access, Excel) a plus. Experience in leading teams to drive changes within the system through development and patient safety improvement projects that meet the needs of the stroke patient population. - Knowledge & Skills - Requires thorough knowledge of clinical care practices, procedures and techniques required to meet targeted patient population. - Working knowledge of computer hardware and software including desktop applications, Clinical Data Management applications and tools. - Requires working knowledge of research methodology. - Demonstrates effective analytical and problem-solving skills. - Demonstrates proficiency in nursing assessment skills. - Demonstrates clear, effective communication skills, including verbal, written, and listening skills. - Demonstrates well developed interpersonal skills necessary to promote and maintain cooperative, courteous, and sincere relationships with patients, family members, physicians, staff, and the public. - Requires ability to independently prioritize and organize work activities and work effectively under pressure. - Requires ability to identify and utilize appropriate resources. Demonstrates computer literacy and the ability to effectively use word processing, spreadsheet, electronic health record, and presentation software. - Requires strong analytical and critical thinking skills necessary to compile, analyze and process clinical information. - Working Conditions - Must be able to deal with varying levels of stress and deadlines. - Requires travel to referral sources, satellite and other locations. - Travel required. - Clear communication and speaking voice for telephone speaking required. - Physical Demands - Requires the physical ability and stamina (i.e. to walk/stand for prolonged periods of time, push carts/wheelchairs up to 50 pounds, to position/lift patients at a maximum of 35 pounds unassisted, over 35 pounds requires assistance, provide CPR, etc.) to perform the essential functions of the position.
