RNs!Responsible for ensuring that PACE nurses, MA and aides provide
quality care across the PACE continuum per licensure and program
policy and regulation. Works under the direct supervision of the
Medical Director (MD). The Clinic Supervisor demonstrates
independence in the application of advanced healthcare knowledge
and skill and delivers direct care to participants.Key
Responsibilities: Responsible for direct reports performance and
growth. Ensures competencies, licensure, hiring, coaching and
disciplinary processes are completed per policy/regulations.
Identifies performance issues and works with MD to address Holds
clinic staff accountable for professional conduct and policy
adherence on a daily basis Ensures all clinic staff are adequately
trained to deliver the highest level of care for PACE participants
in collaboration with the Medical Director. Ensures all clinic
staff have adequate supplies and equipment to perform their duties
in collaboration with the Medical Director. Identifies and refers
potential quality concerns to Medical Director. Completes
documentation related to feedback and accountability discussions
with direct reports in accordance with organizational policy in
collaboration with Charge Nurse. Recruits, employs, and retains
qualified personnel to maintain appropriate staffing levels.
Collaborates with Medical Director on regular basis to implement
continuous process improvement for clinic operations. Responsible
for On-Call services, staffing, monitoring and ongoing process
improvement. Serves on on-call rotation. Coordinates with Center
Manager on all program activities related to care planning and
implementation. Coordinates with Center Manager on all activities
related to new participants and current participants to ensure
service excellence. Responsible for ongoing clinic process
improvement to ensure excellence with care delivery. Demonstrates
active participation in quality improvement processes in the clinic
and program-wide ensuring compliance with quality standards
including but not limited to infection control process and provider
monitoring process. Audits medical records assuring accuracy,
completeness and compliance with licensing regulations,
certification standards, and legal and ethical standards for direct
reports. Implements and ensures compliance to program policy and
procedures to ensure service excellence and employee satisfaction.
Promotes goals of PACE of Southwest Michigan in the community, the
facility, referral agencies, regulatory agencies and other
stakeholders. Actively and purposefully participates with
leadership to build organizational excellence. Is an active member
of committees required by applicable laws, regulations, or request
of the CEO or Medical Director Adheres to the organization's
Mission and Vision. Demonstrates leadership behavior that is
ethical and transparent. Participates in the development of a plan
of care with the Interdisciplinary Team. Coordinates and supervises
all nursing care for participants Demonstrates active participation
and leadership in clinic related QIP committees and work groups to
include reporting to QIPSC Identifies and refers potential quality
concerns to Utilization Review Committee. Weekday and weekend
on-call rotation..Requirements:Specifications:Education: Graduate
of an accredited school of nursing with an RN degree.Experience: At
least 2 years' experience, preferably in either home or community
health. Supervisory experience required. Minimum of 1 year
experience working with the frail elderly population.Number and
Type of Employees Supervised : Clinic Staff (Nurses, Aides, MA's,
Clinic Clerk)Licensure, Registry or Certification Required: Must
possess a current, unencumbered, active license/certificate to
practice in the State of Michigan. Valid MI driver's license.
Possess current CPR certification by AHA or willing and able to
obtain as offered by PACE of Southwest Michigan.PM20
