Job Description:

Responsible for the overall physical therapy program at PACE of Southwest Michigan. SpecificationsEducation: Graduate of accredited Physical Therapy program Experience: Minimum of two (2) years' experience working in a clinical setting as a PT; One (1) year working with the frail and elderly populationNumber/Type of Employees Supervised: None Licensure, Registry, or Certification Required: Active and unencumbered Physical Therapy license in the state of Michigan, valid MI driver's license and vehicle, CPR certification. To improve efficiency and reduce fraud and abuse, a National Provider Identifier (NPI) number is required. Special Training: Only acts within scope of practice. Meet a standardized set of orientation and/or individual competencies for the specific position description established by PACE of Southwest Michigan, and approved by CMS before working independently. Immunizations: Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact. Specific Requirements and Considerations: May be subject to exposure and handling of infectious waste, diseases, conditions, or hazardous chemicals, etc., including potential exposure to TB, AIDS, and/or Hepatitis B. Ages of Patients Rendered Care: Adult, GeriatricKey Responsibilities-- Conducts and documents initial assessments and reassessments of participant's condition and functional status-- Assesses need for adaptive and/or durable medical equipment, specialized shoes, and prosthesis and makes recommendations to the Interdisciplinary Team (IDT)-- Initiates order for equipment and performs quality checks-- Assesses all newly issued orthotic shoes, braces and prosthesis for appropriateness and proper fit-- Assesses the need for physical therapy services provided at the center, followed by recommendations to the IDT and obtains written authorization for services approved-- Plan, develop and implement plans of therapy-- Participates in the discharge planning of participants from hospitals, SNFs and ALFs-- Provide training and education to all involved in the treatment plans of the participant that can be performed by them-- Functions as a member of the IDT, attending meetings and developing plans of care-- Communicate participant changes to IDT members-- Participant in enrollment process including care planning-- Active participation in QIP processes-- Oversees restorative program-- Performs minor DME repairs-- Initiates home modification requests indicated by IDT-- Completes home visits when directed-- Abides by all established PACE of Southwest Michigan policies, rules, and regulations-- All other duties as assigned Skills and Knowledge-- High level of autonomy is required while reporting to the Center Manager-- Excellent attention to details-- Excellent organizational skills-- Excellent verbal and written communication skills-- Ability to work a flexible work schedule-- Able to work within the IDT setting-- Knowledge of physical, mental and social needs of frail older adults-- Effective skills in physical assessment and chronic disease management for frail older adults-- Exceptional interpersonal skills-- Able to utilize computer skills in the workplace Physical Demands PM20
