Position Summary: MDS nurse will conduct, oversee and facilitate
the RAI process with established standards of practice and federal
and state requirements. MDS works with the interdisciplinary team
(nursing, therapy, dietary, social services, activities, etc.) to
develop, revise, update, and maintain a comprehensive care plan and
ensure that compliance is maintained with state and federal
guidelines. MDS is responsible for attesting to the completion of
the Minimum Data Set (MDS), which is the key driver for Medicare
payment and many Medicaid reimbursement systems. MDS serves as the
expert resource for the Patient-Driven Payment Model (PDPM) and is
responsible for complying with ethical standards when setting ARDs,
completing assessments, and upholding Medicare requirements.

Responsibilities/Accountabilities:
- - - - - - - - - Oversee functions related to MDS 3.0 both
regulatory and financial including scheduling, ensuring timeliness
in completion and submission, CAA and comprehensive care plan,
reviewing and correcting validation reports, monitoring for
significant change and/or IPA
- - - - - - - - - Attend Medicare/Medicaid meetings, and care plan
meetings as designated
- - - - - - - - - Assist to schedule and/or open forms for OBRA and
PPS as needed
- - - - - - - - - Will assist to query, clarify, and obtain
accurate ICD-10 codes from the physician or physician extenders,
input into the Medical Diagnosis sheet, and sequence as required
for billing
- - - - - - - - - Update ICD-10 codes quarterly or on
re-admission
- - - - - - - - - Responsible for completion and tracking of the
Medicare Cert/Recert and uploading of forms weekly
- - - - - - - - - Review PPS and case mix data for assigned region
to ensure appropriate utilization from medical record
documentation
- - - - - - - - - Be well versed in Medicare skilled care
requirements
- - - - - - - - - Provide the MDS IDT and/or other facility
staff/consultants with monthly OBRA schedule
- - - - - - - - - Update the comprehensive care plan and designated
forms quarterly
- - - - - - - - - Responsible for the use of clinical technology
applications
- - - - - - - - - Provide IDT training and cross training in the
RAI process -
- - - - - - - - - Focus and anticipate the impact on the following
metrics (not all inclusive):
o - - MDS, CAA, and Care Plan Completion and Timeliness
o - - Medicare Meeting Process to include Utilization Review,
Weekly Medicare Meeting and Triple Check
o - - Medicaid meeting process
o - - QRP reporting
o - - Appropriate Length of Stay
o - - Correct Reimbursement for care and service provided
o - - Five Star Ratings
- - - - - - - - - Participate in Quality Assurance and Performance
Improvement program
- - - - - - - - - Facilitate audits to evaluate accuracy,
timeliness, and compliance of MDS related systems as indicated
- - - - - - - - - Ensure resident care and clinical practice meets
acceptable standards as defined by nursing practice acts, JCAHO
requirements (where applicable) and state and federal
requirements
- - - - - - - - - Plan, organize, prioritize, be detailed and meet
deadlines
- - - - - - - - - Maintain HIPPA compliance
- - - - - - - - - Foster effective working relationships
- - - - - - - - - Check email/voicemail correspondence daily
- - - - - - - - - Other duties as assigned

Physical Requirements and Working Conditions
- - - - - - - - - Sitting, standing, bending, lifting, and moving
intermittently during working hours
- - - - - - - - - Subject to frequent interruptions
- - - - - - - - - May be required to lift residents, medical
equipment, and supplies
- - - - - - - - - Subject to exposure to infectious waste,
diseases, and conditions
- - - - - - - - - Must possess sight/hearing senses or use
prosthetics that enable these senses to function adequately so that
the requirements of this position can be fully met

Specific Educational/Vocational Requirements:
- - - - - - - - - Minimum of Associate Degree in Nursing
- - - - - - - - - CMAC or RAC-CT certified preferred
- - - - - - - - - Minimum of 2 years as a Licensed Nurse

Job/Technical Skills:
- - - - - - - - - Good technology skills and experience with basic
business software
- - - - - - - - - Excellent verbal and written communication
skills
- - - - - - - - - Knowledge of EMR systems or ability to adopt with
sufficient orientation
- - - - - - - - - Knowledge of state specific post-acute
regulations and guidelines
- - - - - - - - - Knowledge of RAI process
