*Up to $12,500 Sign-On Bonus!**Registered Nurse (RN)*-Are you looking for *flexibility *and *autonomy *in your work?Are you ready to advance your career with one of *Indiana's top home health providers*?Join Elara Caring as our next *Registered Nurse* (RN)-*Registered Nurse RN**Salary :* $75,000 - $85,000/yr + up to $12,500 sign-on bonus*Multiple Locations:* South Bend, Granger, Mishawaka, Osceola, Plymouth, and Bremen*Travel / Mileage reimbursement *in addition to base earnings/pay-*Why Join the Elara Caring mission?** Youll work in a collaborative environment* Youll be rewarded with a unique opportunity to make a difference* Outstanding compensation package* Medical, dental, and vision benefits after 30 days of employment* 401K match and paid time off for full-time staff* COVID-19 Prepared with Personal Protective Equipment and precautions-*As a Registered Nurse Home Health, youll contribute to our success in the following ways:** Ensures that all activities performed align with the vision of Elara Carings board of directors, executive team, and the leadership of the Home Health team.* Travel home to home within your coverage area and provide one on one direct patient care.* Maintains patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule.* Assesses assigned case load of home health care patients and families to identify the physical, psychosocial, and environmental needs of patients as evidenced by documentation, clinical records, Interdisciplinary Team reports, after hours reports, and on-site evaluations.* Assumes primary responsibility for caseload, including assessing, planning, coordinating, implementing, and evaluating the plan of treatment.* Assesses patient needs and obtains data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status and incorporate that data into the plan of care.* Makes the initial evaluation and re-evaluates the patients nursing needs during each visit.
