Medicine Physician (MD or DO) to join a dynamic team in South Bend,
Indiana. This role offers an opportunity to make a meaningful

you're a seasoned provider or a recent graduate, this is your

with integrated services across medical, dental, and behavioral
health disciplines. Key Responsibilities: - Provide comprehensive
outpatient primary care to a diverse patient population -
Collaborate with a multidisciplinary team including physicians,
nurse practitioners, dentists, and behavioral health specialists -
Maintain accurate documentation using eClinicalWorks (eCW) EMR -
Participate in quality improvement initiatives and team-based care
- Support and embody the mission of providing compassionate,
affordable care to all Work Schedule: - Monday to Friday daytime
hours (no evenings) - Four-day workweek optional - No weekends
required - Major holidays off - Dedicated administrative time

Qualifications: - M.D. or D.O. from an accredited medical school -
Completion of an accredited residency in Family Medicine or
Internal Medicine - Board-Certified or Board-Eligible - Eligible
for medical licensure in the state of Indiana - Active DEA license
(or ability to obtain prior to start) - Strong commitment to
community health and serving underserved populations - Prior
experience or interest in working within FQHC, CHC, or PCMH
