compassionate and skilled Internal Medicine Physician (MD or DO) to
join a dynamic team in South Bend, Indiana. This role offers an

Responsibilities: - Provide comprehensive outpatient primary care
to a diverse patient population - Collaborate with a
multidisciplinary team including physicians, nurse practitioners,
dentists, and behavioral health specialists - Maintain accurate
documentation using eClinicalWorks (eCW) EMR - Participate in
quality improvement initiatives and team-based care - Support and
embody the mission of providing compassionate, affordable care to
all Work Schedule: - Monday to Friday daytime hours (no evenings) -
Four-day workweek optional - No weekends required - Major holidays
off - Dedicated administrative time included in weekly schedule

community health and serving underserved populations - Prior
experience or interest in working within FQHC, CHC, or PCMH
