dedicated team as a part-time Advanced Practice Provider (APP) and
make a meaningful difference in the lives of individuals
experiencing homelessness. Position Overview: We are seeking a
skilled and compassionate Nurse Practitioner (NP) or Physician
Assistant (PA) to provide patient-centered, trauma-informed primary
care as part of a multidisciplinary team. This role focuses on
addressing acute and chronic health conditions, promoting health
equity, and delivering holistic care to individuals facing complex
social and health challenges. The Advanced Practice Provider (APP)
provides quality primary healthcare services to special
populations, emphasizing the promotion of health and the prevention
of disease. This role addresses society's demand for accessible and
affordable healthcare. The APP works as an integral member of a
multidisciplinary team, managing acute and chronic health issues
while collaborating with physicians, nurses, and other healthcare
professionals. Individual accountabilities and work volumes will be
established annually. Essential Duties and Responsibilities:
Facilitate participant engagement and entry into the healthcare
system by assessing primary healthcare needs, performing physical
examinations, and ordering/performing pertinent diagnostic tests.
Provide coordinated and continuous primary healthcare to clients
across the care continuum through goal setting and care planning.
Prescribe treatment modalities ( based on licensure) to meet client
health and illness needs. Educate and counsel clients to foster
therapeutic and caring relationships. Manage and evaluate care
plans, modifying them as needed to meet individual client needs.
Collaborate with multidisciplinary teams to provide high-quality
care and community services. Coordinate with team members to ensure
continuity of care. Participate in quality assurance activities,
including protocol development, peer review, and evaluating
standards of care. Maintain clinical competence through ongoing
education and collaboration with colleagues. Document comprehensive
and accurate data in client records and reports. Adhere to ethical
and legal practice guidelines set by regulatory and governing
bodies. Participate in professional development and mentorship
activities. Qualifications: Education and Experience: • Physician
Assistants (PAs): Active Illinois PA licensure required. Graduation
from an ARC-PA-accredited program. Preferred family practice
provider. Successful completion of the PANCE exam. • Nurse
Practitioners (NPs): Active Illinois Advanced Practice Registered
Nurse (APRN) licensure required. Active Illinois Registered Nurse
(RN) Licensure required Master’s degree in nursing from an
accredited institution. Certification from the American Academy of
Nurse Practitioner Certification Board (AANPCB) or the American
Nurses Credentialing Center (ANCC) Preferred family practice
provider. Certifications and Licenses: PAs: Must maintain National
Commission on Certification of Physician Assistants (NCCPA)
certification and Illinois PA license. NPs: Must maintain national
certification in a specialty area recognized by the Illinois
Department of Financial and Professional Regulation (IDFPR). Both:
DEA license and Controlled Substance License required. Preferred
Skills: Spanish bilingual proficiency preferred. Experience with
Management of Opioid Use Disorder and HIV preferred Documented
experience in protocol development, quality improvement, and
