Job Description:

Client seeks Nurse Practitioner or Physician Assistant for June 18-August 31 at their Corporate Health clinic in Kokomo, IN. Provider will see ages 2+ for acute episodic care, ages 5-12 for sports/camp physicals, and ages 12+ for Primary Care, acute episodic care, physicals, and wellness visits. Dispensary onsite for ages 12 and up. Hours are Monday 7:00am-4:00pm with a 60-minute lunch break, Wednesday 1:00pm-5:00pm, and Thursday 8:30am-5:00pm with a 30-minute lunch break. Shifts are available on Tuesday, Wednesday morning, & Friday at sister clinic in Arcadia (30 minutes away), and Client prefers provider who can work both sites. Provider must be Board Certified and must have 2+ years of clinical experience in a Primary Care specialty to qualify for this assignment. DEA is preferred. Detailed shift dates are June 18, 22, 24, 25, & 29, July 1, 2, 6, 8, 9, 13, 15, 16, 20, 22, 23, 27, 29, & 30, and August 3, 5, 6, 10, 12, 13, 17, 19, 20, 24, 26, 27, & 31.
