PACE of SWMI is seeking an experienced Home Care Supervisor to add

Salary Range: TBDReports to: Clinical Operations Manager or
designee Job Summary: The Home Care Supervisor is responsible for
ensuring that home and community-based participant care is
coordinated, delivered and managed appropriately through PACE staff
or contracted agencies.Key Responsibilities: (*denotes an
age-related skill or task)-- Oversees the appropriateness of home
and community-based services provided by PACE, the utilization of
related resources, and the quality of care provided. --
Participates in the development of a plan of care with the
Interdisciplinary Team, participants, and the family/caregiver.--
Coordination of 24-hour care delivery and the implementation of all
home care services which include PACE or contracted nursing and
personal care services to ensure that quality services are provided
to meet participant needs in all home and community-based settings
per the plan of care.-- Ensures staff monitoring of participant and
family/caregiver ongoing functional/psychological status and
appropriateness of care, according to the plan of care and follows
up appropriately.-- Tracks changes in homecare orders and
communicates changes to appropriate internal staff and external
providers.-- Provides oversight to ensure DME needs are met.--
Builds strong partnerships with contracted home care agencies and
ensures regulatory and quality of care expectations are met.--
Performs regular supervisory visits of PACE homecare nurses and
aides as well as and contract homecare to ensure provision of
quality care.-- Ensures ongoing Home Care Assessment Tool (HCAT)
training and completion by appropriate staff.-- Adheres to HR
policy and procedures related to hiring, coaching, and managing
direct reports.-- Responsible for collaboration with fellow RNs in
administration of the PACE Personal Care Aide (PCA) program, to
include training and supervision of PCAs.-- Reconciles all contract
homecare billing to authorized hours and services. -- Provides
participant/caregiver education as needed.-- Participates in IDT on
a regular basis.-- Demonstrates active participation in continuous
process improvement and quality initiatives.-- Completes PACE nurse
duties, as necessary.-- Ability to assess with critical thinking
skills the physical, psychosocial and home care needs during
initial, periodic, routine and acute assessments conducted
minimally every 6 months.-- Performs weekday and weekend on-call
duties.
Requirements:
Specification Education: Associate degree in nursing and active
Michigan RN licensure required.Experience: At least 2 years'
nursing experience, preferably in either home or community health.
2 years supervisory experience required. Minimum of 1 year
experience working with the frail elderly population in homecare,
community health or nursing homes.Number and Type of Employees
Supervised (optional): Home Care teamLicensure, Registry or
Certification Required: Must possess a current, unencumbered,
active license/certificate to practice in the State of Michigan.
Valid MI driver's license. Possess current CPR certification by AHA
or willing and able to obtain as offered by PACE of Southwest
Michigan. Special Training: Only acts within the scope of his or
her authority to practice. Meet a standardized set of orientation
and/or individual competencies for the specific position
description established by PACE of Southwest Michigan and approved
by CMS before working independently. Immunizations: Be medically
cleared for communicable diseases and have all immunizations up to
date before engaging in direct participant contact.Specific
Requirements and Considerations: May be subject to exposure and
handling of infectious waste, diseases, conditions, or hazardous
chemicals, etc., including potential exposure to TB, AIDS, and/or
Hepatitis B. Skills and Knowledge--High level of autonomy is
required for clinical and management decisions while reporting to
Clinic Operations Manager. -- Excellent problem-solving skills.--

Ability to work a flexible work schedule.-- Ability to multitask.--
Knowledge of physical, mental and social needs of frail older
adults.-- Effective skills in physical assessment and chronic
disease management for frail older adults.-- Able to work within
the Interdisciplinary Team (IDT) setting.-- Able to utilize basic
computer skills in the workplace.-- Able to utilize organization's
electronic medical record system. Physical Demands The physical
